4/21/61
-12:00PM R. McClelland placed in seclusion for the rest of the day and night
for disobeying Mr. Mitchell, repeatedly.

-3:00PM R. Hepburn (Lime Cottage) apprehended while wandering around Hemlock.
He was placed in seclusion and is to remain there until after breakfast on Sat.
4/22/61. At that time he is to be returned to the Lime Cottage by Lime staff.
As per instructions of doc (?) Jackson.
-4:30PM Ben Inabinett didn't take shower. ...When boys had finished showering
we asked him why, he said his back hurt. But he didn't complain in previous.
Results: Seclusion out at 8:30PM. Gave threats about what he would do when he
came out.
-6:30PM A Parker has open sore on Stomach treated with Bacitracin oint. and
Bandage. -8:30PM Ben released from seclusion
4/22/61
-10:30AM R Hepburn released from seclusion back to Pine Cottage.
-6:30PM R. Goddard placed in seclusion for attempting to abscond from outside
play area.

-7:30PM Prepare for bed. Liquid discovered under R. Goddard's bed in seclusion.
Upon questioning, Richard admitted lying on the bed and trying to urinate in
a medicine cup. Richard is to remain in seclusion another day for this offense.
4/24/61
-9:00AM A. Parker put into seclusion for hitting H. Daniels. To be released
4-24-61-8:30PM.
-5:15PM Epps placed in seclusion by Mr. Philips for failure to respond when
spoken to and general (not legible) and stubborness because he had to take shower
with rest of residents. Please do not release Epps until 8:30PM

-12 Midnight Because of unfamiliarity with the circumstances concerning Arthur
Parker and lack of information as to his stay in seclusion, I released him tonight.
-5:10PM 3 Boys in seclusion John Lewis to stay in for 3 days not to be included
in population. James Dean, Russel Sharrow the two will come out tomorrow at
7PM. Clothes beside desk.
-Note: Arthur Parker had seizure. 5PM

-7:00AM Residents awaken and taken outside for P.T. Exercises, Medication
-8:30AM D. Spitzhoff released from seclusion. His lips were swollen as a result
of his picking them.
4/28/61
-10:AM D. Liberto (sp?) was put in seclusion for stealing and eating my sandwich
in the employees lounge. I put him in seclusion for the day because of this.
He evidently became very disturbed over this and when we went into give him
his meal he had torn all his clothes to shreds and had tied a shoestring around
his neck. He was choking to death and the string was so tight he wouldn’t
move his head. Mr. Duck (sp? not legible) and I had to cut it off with scissors.
We took all his clothes off him, and later when we checked him he was trying
to tear his penis off with his hands. He also refused to take his medication.

-3:00PM R. Goddard released from seclusion. ( note: he was in seclusion
for 6 days for attempting to abscond from outside play area.)
-Note: John Lewis is to be released to return to his cottage this morning.

-7:40AM Showers and preparation for breakfast Arthur Parker struck Liberatti
from behind; placed in his room for remainder of the day! Liberatti placed in
seclusion for swinging at Parker.
-11:05AM Liberatti and Parker apologized to each other on their own taken out
of seclusion at 12 noon.
-Note: R. Goddard in Seclusion for three days as a result of trying to strike
me in a tussle over a paddle tennis bat
-Note: J. Epps put into seclusion for (3) days advantage taken of W. Gordon.

-Note: James Epps given 3 more days for coming at Mr. Phillips not to be released
until 5/11/61.
-11AM Certain residents got mad. Epps still in terrible mood toward fellow residents
and (not legible) Goddard still mad at the world.

-9:00AM Note: Burley given three days for attempting to smuggle cigarette butts
into seclusion. An investigation was made of his room and a match strike was
founded.
-9:20AM Parker had a seizure and the proper percautions were taken
-5:00PM Note: A piece of metal was found in R. Burley’s seclusion room.
Burley used this metal to take the screws from his radiator cover. He insists
that he didn’t do this. Metal was left on Mr. Hodge’s desk.

-8:00AM Note: R. Goddard released from seclusion
-9:00PM Note: All boys in Seclusion are to sleep in their regular rooms &
return to Seclusion during the day. Mr. Hodge discussed the possibility of these
boys attending classes that may prove beneficial to them

-12:15AM Note: Epps and Burley in conversation. I cautioned them to quieten
down. Burley attempted to steal a cigarette while I was preparing the shower.
This was while he was out for a shower.

-7:35AM Miss Green called and stated that John Frazier was to be admitted. She
also stated that he is not to be let out of his room for any reason.
-7:45AM John Frazier admitted to seclusion.

-3:00PM Note: Frazier was quiet during the morning and ate his meals in good
humor.
-8:05 Mr. W.C. Jackson in seclusion with John Frazier.

-10:30PM V.O.T. John Frazier officially in Hemlock, and is to remain (Mr Jackson)

-Note: J. Williams and R. Burley aren't allowed to smoke for one week. They
were caught smoking during free rec. They smoke again after supper May 26th!
-Medicine wasn't received today. We borrrowed dilantin from Elm Cottage.
-A. Parker struck F. Caitlin in the head with a baseball bat. Parker stated that
he did this because Catlin attempted to change the radio dial from rock &
roll music to the baseball game.

-10:15AM Note: R. Burley, Sheldon abscond by taking keys from front office desk
and leaving by front door. Chase was given however not caught.
-1:20PM A. parker had violent seizure had to be taken to room to lie down
-2:25PM Note: R. Burley returned to cottage by Mr. Anderson.
-9:00PM Note: R. Burley is not t be let out of his room for any reason until
we hear from Mr. Webster. Orders of Mr. Webster.
5/21/61
-9:30AM Raymond Burley broke the glass out of his cell door trying to abscond!
In a search of Burley’s person I found a metal object on him which he
was trying to unlock his door with. I removed his pajamas and robe as a safety
measure because he is talking about taking his life.
-12:35AM R. Burley complained that he had a wet sheet. His request for another
was denied.

-6:00AM John Matika arrived with Mr. Pinkney was given shower and put in seclusion
-Note: R. Burley had matches in his room had hid them in the radiator cover

-3:00PM Boys in seclusion cleaning entire seclusion area.

-2:45AM Note: McClelland is seriously disturbed-had bad nightmares and prayed
aloud much of the night. He has mentioned suicide. He wants to see his parents
very much. He deserves constant watching and care. Emotionally he is almost
at the breaking point. He appears to be in need of psychiatric as well as other
medical care.

-9:45AM R. McClelland in bed hollering and crying when I checked and asked him
what was bothering him he stated he was dying. I asked him what brought this
on, he said his feet was cold and this meant you were dying. I talked to him
for 15 minutes. Then he asked me was his father in jail I asked him who told
him this. He said no one. After this I locked him in his room and told him I
would check with him later at this time he stated that he would be dead when
I come on duty tomorrow.
-3:50PM Roger Poore Holly Cottage admitted to Hemlock.
-6:00PM Door to seclusion in Roger Poore’s room jammed. Locksmith called.
-11:00PM Matika did a lot of hollering between 9&10:30PM after this he went
to sleep. Made periodic check on Roger Poore was sleeping very peacefully.
5/30/61
-8:00AM All seclusion boy shower and brushed teeth except R. Poore. Med given.
Resident cleaned rooms and general house cleaning.
-9:30AM Joe William – Epps fighting in dining room over food. Gordon want
to fight Epps for hitting Joe.
-10:30AM R. Goddard & Bennie fighting in play area. Mr. Duck took Richard
to hospital but no doctor.
6/2/61 -8:00AM
Note: Freddie Catlin place in seclusion in for 3 days to be released on 6-5-61
at 8:30PM. He is continually cursing the residents and employees.
6/3/61
-11:50AM Parker struck Moticka in the latrine this could have been avoided if
this had been closer supervision-Mitchell
Liberatti was caught just after completing an act of perversion with Moticka.
Liberatti & Parker no smoke break rest of day. Mitchell

-1:00AM Note: R.Poore became restless and engaged in handclapping and other
noises. I attempted to quieten him down but failed. He the attempted o break-oout.
The guard was called and was soon on the scene. The guard had the door opened
but soon found that Poore was beside himself. He requested the door locked.
Upon checking with the night supervisor Poore was permitted to walk up and down
the hallway in the seclusion area. He went back to his room and began tearing
the tile from the floor, etc. He continued to be restless and noisy. The guard
remained for sometime.
-4:20AM Note: Poore has torn up the room to the extent he may need to be moved
to a new another
6/4/61
-11:15AM Mr. Jaclson called for one of the employee to come to Elm to go with
to go with them to popular to pick up Harry Brown a transit for Hemlock until
further notice.
-Note: The new seclusion form is to be kept in duplicate and the original copy
in the supervisors office each morning. Each time the resident is seem it is
to be recorde.
-11:45AM Harry Brown picked up from Poplar as ordered by Mr. Jackson. Showered,
shaved,, and feed before placed in seclusion

-8:30AM Arthur Parker had a rather violent seizure.
-9:00PM Note: Fred Catlin released from seclusion in original room.
6/6/61
-12:45PM Mrs. Stahlman and Mrs Ness visited the cottage and were taken on tour.
They were quite impressed with building residents & program.
-9:00PM Note: Pheno given to Rodger Poore at 7PM -Note: J. Matica had B.M. in
dining area.
6/7/61

-9:15AM Fred Catlin placed in seclusion for going through double doors without
permission-Mitchell
-3:00PM Note: David Johnson in Hemlock Cottage to be included in cottage population
he was brought by Mr Whitney, Mr Glenn. Clothes to comem later. Was put in Fred
Catlinís room because of crowded seclusion area. Freddie removed to his regular
room.
-7:00PM Medication not given as ordered due to shortage on R Poor's thorazine.
-8:30PM Boys given talk about sliding regulations within cottage all told about
not following orders not lining up correctly, shower procedure, smoking procedure,
the leaving of their rooms dirty after falling out at 3:30PM for evening sessions.
Note: These are the things to be checked by both shifts lining up in a file
not leaning on wall. Everyone having shoes in front of door heels to wall. Showering
quickly but thoroughly and everyone washing their teeth and the correct following
of orders. If these things are checked by both shifts we wouldnít be continuous
to have to chew these students out for the same thing over and over.
6/8/61
-7:30AM Note: Morning shift does not have trouble with resident lining up or
leaving dirty room. However the morning shift would appreciate it if the other
shifts would have resident get (not legible) clothing dry. There are a lot of
ways to be wasteful.
-8:55AM A Parker hit Freddie Catlin for no good reason and put a cut over his
eye. First aid given and Parker put in Seclusion for the rest of the day. -12
Noon Pheno is out-I suggest that some be gotten soon!
-2:00PM Burly room found to contain matches, cigarette butts and pin taken from
door hinge
-3:00PM Note: Fifty Pheno tablets brought to Cottage by Mr. Pinkney.
-8:30PM Note: Boys after instructions seemed to be lining up in a correct file
without any trouble at all.

-9:00 Breakfast P. Poore tore radiator cover off wall report to have man come
fix it R. Poore put in another room until his radiator cover is fix.
-2:30PM Note: R. McClelland door found open while he was lying in bed asleep.
-11:00PM Note: R. Poore & J. Mathis room's cleaned, Bleached, sprayed out.

-10:45PM Note: Matika tore up a part of the tile floor in his seclusion room.
He stacked the parts in the camode.

-11:00PM Note: Robert Lee Jones to Hemlock by order of W. C. Jackson for medical
purposes has temp of 104. Should be given plenty of liquid throughout the night
to curve the temp.

-7:00AM Resident awaken and given p.t. seclusion bay bedding taken.
-10:15AM Note: D. Kieth to be released on 6/12/61 0r 6-13-61.
-2:15PM Nurse Ryan visted with R. Poore and there is new medication.
6/13/61
-10:30AM F. Catlin, while playing softball collided with Spitzhoff and riocheted
off of the wall. Spitzhoff remained stationary, Catlin was shook up. Put in
the room to rest and given 2 A.P.C.'s.
-1:20PM Man came to fix Radiator in Seclusion area and to replace all screws
missing from other radiator left at 1:55PM.
6/15/61
-10:00PM R. Poore pajamas were taken from him, he put them in the face bowl,
turned the water on, and just splashed them as if he was washing them., the
hall way in the seclusion area, and around the desk had to be mopped up, and
his room also.

-3:30PM Hemlock Cottage presented a womanless wedding reception and cottage
tour followed.

-8:50PM Note: John Harris admitted to Hemlock seclusion as a transient. by order
of Mr. Barber. He's from Elm cottage
-9:00PM Note: Matika tore up a portion of the floor in his seclusion room.

-2:40AM Note: Epps was restless, Kept complaining of inability to sleep. I allowed
him to talk to me apoximately 2 hrs. I believe that Hemlock's Program has reached
him as far as possible; and for his own sake, he should be considered for some
other situation.

-8:00AM Note: David Johnson caught "urinating" on his radiator smoke break taken
until 8AM 6-28-61. Movie priveledge taken for today
-12:45 Note: Freddie Caltin hand to be soaked in Hot epson water morning and
night. He is also to be taken to medical unit for X Ray Monday AM.

-10:00AM Mr. Jackson called cottage informing me to prepare cell for one of
Elm's Boys to spend 203 days here. He's to be carried as a transient. James

when visited he completely messed up his room.

-7:30AM Matika continues to mess up his room every night, tore up his trousers
and 1 shirt.
-8:45AM Note: Daniel Johnson is to be put in seclusion Room at night. He is
constantly urinating in his radiator. OKed by Mr. Hodges.

-10:45AM McClelland & Maddox had argument and had brief scuffle (not legible).
I use punitive measures to try to alleviate this in the future. (I made them
exchange slaps after 2 exchanges they lost thier stomach for fighting). (Parker
placed in seclusion)
-1:15PM J. Matika taken to the Hospital for X-Ray
-3:30PM Note: Boys out for Drill Instruction with Mr. Chatman. A. Parker's have
left unlocked during rest period.
-3:30PM Note: A. Parker's smoke break taken for 7/1/61 for not apprising staff
of the fact that his smoke break had been taken 6/30/61

-1:20PM Movie held in the dining room for the residents who completed their
work-work for the one's who didn't completed their work or goofed off.
-7:15PM Fred Catlin in Seclusion disobedience, agravating periods and continues
to mumble after being reprimanded

-5:00PM Supper-Spanish rice cheese peaches. Smoke break. Because of the intense
heat the hose was turned on in the play court
-7:30PM Note: Water was turned off in R. Poore's room because he continues to
stuff his night shirt in the sink causing floods in the corridor.
-10:00PM Roger Poore clapping very loudly in his room however couldn't make
him stop.

-7:30AM Maddox & McClelland are still feuding with each other. They were made
to do 20 push ups in the shower room. (dangerous drug sheet wasn't signed at
3:00PM 7/2/61 for Roger Poore)
-9:00AM R. Burley release from seclusion by Senior Counselor. 7/3/61-He's in
R. 15 (editor note: Burley put into seclusion on 5/20/61 for absconding)
-5:30AM Burley says he has a bad stomach and needs to see a doctor.
7/4/61
-11:35PM Note: Burley was sick on the stomach and vomited in room. Mrs. O'Brien
sent Mrs. Curry to take temp, 98 and administer medication.
7/5/61
-7:30PM Bingo game. Arthur Parker had to be placed in his room for attempting
to pick fights with everyone in his path.

7/6/61
-6:30AM Note: Matika flooded his room and the one next to it last night. I suggest
that the water be cut off in his room.
-3:00PM Note: Mr. W.C. Jackson called to inform the staff that social service
has given Vernon Maddox permission to go home on visit Sat-Mon 7-8-61 thru 7-10-61
because his father is deceased. He is to be picked up by his parents or either
transportation is to be arrange sometimes Saturday. Tomorrow 7-7-61-A.M. Vernon
is to be taken up to Pine Cottage and fitted for dark suit and other things
needed.
-9:30PM Note: Maticka Flooded the whole outside and inside halls, rooms, & bathroom
-12:40AM Note: It took Mr. Glen, Maddox and I about 2 hours to clean the water
from the hall floor. Mrs. O'Brien suggested that Mitika be placed in regular
room for the night. He was placed in Johnson's room. Miss Green suggested that
only one resident be let out to help. Hence thelong time to clean the floor.
Something will have to be done about this promptly or water will be all over

-10:05AM Mrs. Robinson visited cottage. talk to Burley, Maddox, Catlin, McClelland
abd Parker.
-10:45AM Mr. Smith took Maddox to Pine Cottage to be fitted for a suit & o get
a shirt and tie.
-10:50AM Note: Mr. Hodge says to leave the water off ib Seclusion area until
further notice. CUT ON ONLY WHEN NEED.
-9:00PM Note: William Gordon failed to shower this evening he is always messes
around until it is time to turn it off I think he does it purposely he doesn't
like to shower period.

classes.

-8:30AM Note: The toilets in R. Poore room and Metika room were stopped up had
to call the plumber because when we flushed the toilets water ran all over the
place.

-11:00AM J. Williams & H. Dabiels got into a fight in kitchen Don't know what
started it put Joe in his room for a while he was upset.
-3:00PM Note: All Clothing is to be taken up at night from Poore and Maticka.

seenlurking in that area by operator who called cottage
-9:00PM Note: Leroy Johnson in room in cottage. checked in at 5PM by guard.
-10:45PM Note: W. Chatman & Hill enroute to Wash for pick-up of W. Sheldon.
Mr. Webster's orders are for Sheldon to be placed in seclusion for an indefinite
period. He is to be let out only for showers.

-11:00PM Note: We are all out of (not legible) medication for Maticka
-1:30AM Note: W. Chatman returned W. Sheldon to Hemlock cottage. He was stripped,
showered and placed in seclusion.

-9:20AM Note: Cottage Inspection. Physicl appearance excellent. Considerable
evidence of program improvement.
-9:20AM Note: On Mr. Webster & Mr. Shea suggestion the hall to the seclusion
area should be left open at times to allow the air to pass through.
-11:00PM Insert: Group separated into 2 parts Healthy and the sick at 3:30PM
everybody except 7 boys had colds and flatly refused to participate in outdoor
shower programs. We felt that this had happened to many times and we decided
whats the best way to handle the program our way or their's. We decided on our
way. We allowed 7 participant to watch T.V. after classes were over and the
others showered given smoke break and put to bed. starting 7:30PM until 8PM
non-participants shouldn't be allowed to reap the same benefits as the participants,
those who were put to bed were given cold tabs to help them rid the cold.

-1:00PM Note: James Adams admitted to Hemlock Cottage by orders of Mr. Webster.
He will remain over night and be released in the morning. He is from Pine Cottage.

-12:45PM Note: Fred Catlin gave me a book of Matika's that he said Raymond Burley
put in his bath robe.
-6:00PM Note: Earl Hundly brought to Hemlock by Mr. Davis, Chatman, Jackson
WC, Mc Geachy for preparation to go to hospital 7-18-61

-9:30AM Note: please call hospital before phen. bar. runs out-so as to have
some on hand while waiting for a new supply.
-11:00PM Note: Liberati, Matika to the bathroom, Matika sat on Hopper 15 min
and did nothing. He went back to his room and urinated in the training pan standing
up.

-11:00PM Note: D. Liberati put in seclusion for two days and is to miss all
of his privileges for two more days, because of attempted abscond. punishment
started immediately

-9:30AM Note: E. Huntly refused his medication this morning scratch and bit
Mr. Mitchell on his hand when he made an attempt to administer his medication.
